The following fields require that you enter your information in a specific format:
1) Name - Please enter your full name as it appears on your health card (First and Middle Name(s) and Last Name)
2) Date of Birth - If your birthday is June 9, 1983, this would be entered as 09/06/1983 (DD/MM/YYYY).
3) Ontario Health Card Number– Please only include the 10 digits on your health card and do not include any dashes, spaces or version code.
