MyChart’s Terms and Conditions of Use


Mackenzie Health offers MyChart for non-urgent access to your health care information. This online tool provides access to some portions of your medical record through a secure web portal. I understand at this time, MyChart does not provide access to my entire/complete medical record.

MyChart does not replace your relationship with your physician.

In the event of an emergency, visit your closest emergency department or dial 911.


Use of MyChart

With MyChart, you will have online access to:
• Check in online 7 days prior to your next appointment
• View past and upcoming appointments
• View preparation instructions for upcoming appointments
• Access education materials related to your upcoming visit
• Complete pre-visit questionnaires
• Cancel appointments (available for select clinics)
• Update your demographics at any time (includes address, phone number, primary care physician, and more)
• View lab and test results
• Diagnostic Imaging reports and images including X-rays, ultrasounds, CTs, MRI
• Clinical notes on your care and treatment
• Access your list of conditions or medical concerns diagnosed at Mackenzie Health as well as your prescribed medications and known allergies
• Phone and Video visits (available for select clinics)
• Appointment management including self-scheduling, rescheduling and wait list sign ups (available for select clinics)
• Access billing information and statements online**

* Although patients can view and cancel appointments using MyChart, some clinics will require the rescheduling of appointments be done directly with the respective clinic via telephone or in-person.
**By creating a MyChart account, you will be auto-enrolled in our paperless billing system meaning you will no longer receive printed statements by mail. You can unenroll at anytime by accessing your Billing Account Summary and clicking on the corresponding link.


Video Visits Terms of Use

At Mackenzie Health, privacy is one of our top priorities, so we want to assure you that virtual visits are made in a privacy protective setting.

To enhance security and confidentiality, we recommend that you use a personal device for your visit and not a cell phone/computer/laptop owned by an employer or third party. A private connection at home or somewhere you trust should be used rather than, for instance, a coffee shop.

Mackenzie Health uses a secure and privacy protective tool but, because of the nature of cybersecurity including use of email, it is not entirely without risk. Please review the following measures to protect privacy and support a positive virtual visit.

I understand and agree that:
• Information about my health will be collected and used during a virtual visit to provide care and may be disclosed as necessary to provide continuity of care e.g. to my primary care physician.
• I will not have anyone present at the visit whom I do not wish to have present.
• In accordance with Mackenzie Health privacy policies, no audio or video recording of the visit will be made by me.
• I may withdraw my consent for using email to send a link to participate in a virtual meeting at any time.
• Mackenzie Health may withdraw the virtual care visit opportunity at any time.

I understand and agree that by participating in a virtual visit that I am accepting the risks and agreeing to the above noted terms.

Confidentiality

Mackenzie Health will keep my personal health information confidential. In Ontario, the confidentiality of personal information (including, but not limited to, health information) is protected under law. Our staff and physicians are subject to strict standards on maintaining your confidentiality. By accessing your medical information with MyChart, you share this responsibility.

Personal Identification Number (PIN) and Password

I understand that Mackenzie Health will send me a unique access code (PIN) to enable access to MyChart.

The first time I log into MyChart, I will be required to create a unique User ID, along with a password.

I agree to keep my user ID and password confidential. I understand my user ID and password would give access to my personal health information, ability to add to my personal health information and send messages on my behalf. It is my responsibility to change my password if I believe it has been compromised or shared with someone else.

It is recommended that you change your password on a regular basis.

Every time I log into MyChart, my user ID and password will be audited to safeguard usage.

I understand that Mackenzie Health takes no responsibility for and disclaims any and all liability for damages arising from a breach of medical record confidentiality resulting from my sharing or losing my password. I understand sharing my User ID and password or abusing my access privileges, may terminate my access to MyChart.


When Will Records Be Available?

Results are released as soon as they are available (on average between 1 to 14 days). If you do not see your results within 14 days, please call 905-883-2202 from Richmond Hill or 905-832-4554 ext. 2202 from across Vaughan.

Availability

I understand that MyChart may not be available during periodic system maintenance, back-ups or failures. Access is provided on an "as is available" basis and I am not guaranteed access at any particular time.


Restrictions

At this time, MyChart is not available to patients under the age of twelve (except if asked for by legal guardian).
